This form is for results of Blood Tests/ Scans or Procedures ordered by your GP only.
The results of tests that were requested by a hospital doctor will be sent directly to the doctor who requested them, and they will then communicate the results to you.
If you have had tests done that were requested by your hospital doctor and you have not received any results then please contact the consultants secretary directly.
Your GP can only report on results of tests that they have requested or performed for you.
