This form is to enable patients to submit home Blood Pressure readings, as requested by their doctor. Your GP will review and respond via text within 7 days.

For best results, record your blood pressure while sitting down, around the same time each day. Take it up to 3 times, and write down the lowest readings for that day. At the end of the 7 days record your readings in the form here
